Office Hours: Mon to Thu 9:00 to 5:00 and Fridays 1:00 to 4:00. Closed every first Friday of the month.
Drop-in hours for new clients and intakes:

What to bring to your intake worker?
  • Identification card for adult member of the family
  • SSS for the whole family
  • Immunization record of children
  • Birth certificate of children
